Incident Report E-Form
For reporting leaks, spills, fires, or other undesirable events, operators can submit electronic reports to the division. To learn more, click here.
Operators may also amend existing reports previously entered into the system. Original reports should be amended when corrections are needed, rather than creating new reports for the same event.

Utah ePermit - Login
Electronically submit Applications for Permit to Drill (APD), Sundry Reports, Well Completion Reports, and more. This program allows oil and gas operators to electronically submit APDs, Sundry Notices, Well Completion Reports, Workover Tax Credit applications, and Well Logs to the Utah Division of Oil, Gas and Mining. Operators submitting their well logs here as PDF images are no longer required to submit paper copies of their logs. An ePermit registration form (see below) must be submitted prior to using this system.

NOTE: If you encounter problems with ePermit or have questions, please contact us via email at OG-ePermit@utah.gov. Please include the number of the APD or Sundry you are working on, if available.

Utah ePermit Registration Form
Register to use ePermit before electronically applying for permits and submitting reports and well logs. A company must register to use the ePermit system by completing this form and sending it to the Utah Division of Oil, Gas and Mining prior to electronically submitting Applications for Permit to Drill (APD), Sundry Notices, or Workover Tax Credits. The original, notarized form must be submitted. Copies will not be accepted.

* Working from home during the COVID-19 pandemic has slowed the Division's ability to process paper forms. Therefore, we are now accepting an emailed copy of the ePermit Registration Form for processing (please send to reportingOG@utah.gov). Please follow up by mailing the notarized paper form to the address shown on the registration form.

Utah eForm Registration
Register to use eForm to electronically submit production, disposition, gas plant, and injection reports.
Click on the "Need to add a User? Register User" option on the eForm Login page and complete the registration form. Read the "Certification" paragraph. Click on the "Register" button. Once division personnel receive the registration application and approve it, the new user will be notified and allowed to access the system.

Utah eForm - Login
Login to submit oil and gas well production and dispositions, gas plant information, and injection reports.
The Utah eForm program allows oil and gas operators to submit the following monthly reports electronically to the division:
- Oil and Gas Production Reports (Form 10)
- Disposition Reports (Form 11)
- Gas Plant Reports (Forms 13A & 13B)
- Injection Reports (UIC Forms 2 $ 3)
